It's been three months since the squid games had finished, Nam-gyu's left with the guilt of killing multiple people and sooner or later begins to see them in his everyday life, believing that they're ghosts that've come back to haunt him. After failing his attempt to kill himself, he's forced into a psychiatric ward. Unfortunately, he's not going to let himself be tricked by others into becoming vulnerable. He's done that once by accident, and he had to witness multiple die, and had to kill multiple. He won't go through it again.
