Yeah the role that Ty has put the Mikes in is an incredibly cruel one in that they are expected to essentially shepherd their Mikey's through medical torture but ultimately can't really do anything to protect them.
When I first started planning this arc I knew that at least one Mike had to die because I couldn't really threaten something like that and then not follow through with it but I also didn't want to just kill a Latvia Mike for no reason. And as I wrote out my outline for the arc I realized i had two Mikes that could die and that it would make sense for both of them to try to get their Mikeys free from the compound. That bit in particular was important to me because I also wanted to make sure that what happened in the compound mattered. It would have been very easy to write this so that all these Latvia Mikes felt disposable and I really didn't want that. I wanted to drive home the point that their lives mattered and that this time in the compound would have far reaching consequences. It was important to me that their deaths me memorable. And I also wanted to have some dignity in their deaths.
I had been planning the time loop essentially since before I even wrote the first chapter. I wrote the first chapter with the plan that the Mikey who dies in chapter 1 wasn't a Mikey at all but a Mike. I hadn't figured out how or when or why, just that the person Michael kill was (or could be) a Mike and not a Mikey. It wasn't until I started planning this arc that i figured out how to weave that back in and to do it in a way that was tender and meaningful. It's a not a stunt I wanted to pull off lightly. And yeah Michael, Walt, and Edgar are devastated (understandably) but at the same time, Mike Seven was comforted by the fact that he was with Michael and that he knew that while Michael was going to kill him, Michael would be gentle and not allow him to suffer. And Seven took comfort in the fact that his death directly contributes to him and Michael getting together. Medical Ty can be as smug as he likes about it but in the end Seven got to die held by Michael (as did Mike Nine).

As for the temptation of the aneurysm studies, Ty is very smart to offer this while the Mikes are in such a vulnerable position and at a point when they feel so distant from everything in the outside world. Not to mention by this point they've been in the compound for about two months and are all clamoring for some agency. Ty is suuuuuuch a bastard.
