John and Sherlock aren't flatmates. Instead, John gets a job in Bart's morgue, and Sherlock attempts to abscond with body parts.
Which is just not on.
In this world, most of the population has a soul mate. A Vision, usually given around puberty, helps mates find each other. Sherlock and John are old enough that statistics tell them their soul mate must be dead. Statistics were wrong.
Every second that ticks by is a reminder of the deafening quiet Sherlock has left behind. John feels his soul slip out of his fingertips, inch by inch, with every passing day.
At some point, he stops trying to hold on.
Sherlock Holmes has met exactly twenty-four Johns in his life. They have all been the wrong John. He's getting tired of waiting, staring at the inscription on his finger and wondering when his John will turn up, if ever.
