Overprotective fathers are nightmares aren't they? Add in the fact Hitoshi has no parents to take his side, Izuku's dad is the wildly unhinged Ruler of Villains everywhere, and the cards really are stacked against him.
Through the miracle of all miracles though he doesn't end up dead. He's not sure... How, exactly, but here's the gist.

“Are you tired, Tenko?” Izuku asks as he stumbles to the nearest piece of rubble, collapsing as his legs give out to lean back against it.
Tomura immediately gets angry, “What? Don’t call me tha—“
“We’re both dying, Tenko. It doesn’t matter what I call you, or how much I hate you, or how much you hate me.”
In which there is a slumber those cannot wake from. And two children, so very tired.

Izuku is quirkless, but that doesn't explain why he can't seem to stay dead.
He always gets a choice, between two doors. One to move on. One to stay behind (to stay alive).
There’s some days he doesn’t want to come back, and the longer he spends in that room between two doors, he thinks that that part of him is right. He’s still got a list of what he wants to get done first, but when push comes to shove...Izuku will fall first.
(Home has not been alive in a long time).
(Izuku has always been good at saving everyone except himself.
Even in death, he is a miracle).
