If hallucinations or delusions do not inhibit a person's ability to function or cause them distress, do they need to be treated?A life in fantasy might be better than a life pumped full of anti-psychotics and marginalized because of the symptoms of mental illness that persist despite treatment. If the delusions are causing problems, or if awareness that one's perceptions do not match the perceptions of others cause distress, then of course one ought to seek treatment. Otherwise, life is painful enough without looking for more reasons to hurt.

DK. Difficult, emotional story starting with Sherlock's earliest memories of John, his imaginary friend, part of reason he is given childhood diagnosis of schizophrenia. Medication makes John go away, so Sherlock secretly stops taking it, manages to fool his family and teachers for years, until the John in his head tells Sherlock to let him go and start meds again.

University Sherlock discovers cocaine, heroin, lots of sex, LSD once and John reappears for a moment. When Mike brings John to meet Sherlock at Barts, Sherlock recognizes him but John doesn't; of course John has his own history and it didn't include childhood with Sherlock. Sherlock starts to fear he is now completely insane, but John puts his hand on his wrist, says he knows he lives with a madman; they belong together.

This is one of those interestingly conceptual fictions that is essentially plot-less but still manages to convey an incredible amount of emotion. It carries the same sort of mysterious mood as movies like Pan's Labyrinth, where even at the end you're not entirely sure what was real, but you know that it doesn't really matter.
