Dean might have figured out part of it out when he thought Cas was dead (again) after the whole Godstiel/Leviathan mess, and then that it was mutual in Purgatory, but there's still a difference between realizing you're in love with your best friend and oh hey, turns out he likes you back, and actually seeing him apparently trying to find out how far he can get his tongue down your throat.
Of course, if it's not actually your throat, just that of your exact doppelganger, that just makes everything easier.
