Jimmy Novak has lived the past 4 years trying to keep his daughter, and him, from being captured and occupied by the Angels - the Angels had came quick and swift when they brought the redemption on the planet, and have occupied almost all humans in the world. There are only a few pockets of human resistance that remain in hiding. They are out there. They are Jimmy and Claire. They still survive because Jimmy promised to keep them safe.
Jimmy Novak broke that promise 6 years later, after being taken and occupied by the angel Castiel.

Dean makes it out, and only part of Castiel makes it out. With only a reserve level of grace left in his system, Castiel was only able to get his vessel back to Earth, Jimmy.
